Responsibilities
- Ensures all medical records documents are processed accurately and filed timely to enhance timely availability of complete data that facilitates quality patient care.
- Assembles and analyzes medical records following hospital/clinic procedures.
- Performs filing and copying of medical record documentation in a hybrid environment (electronic and hard copy).
- Indexes and imports documents into the electronic medical record; reviews to ensure correct importing.
- Scans and indexes loose reports into the electronic medical record.
- Logs and maintains medical records sent to offsite storage.
- Interacts with physicians and other medical staff for retrieval of records.
- Retrieves medical records for special studies, chart reviews, billing, external reviews and committee meetings within required time frames.
- Tracks, and maintains a log of all patient charts that are requested and/or checked out by other departments.
- Processes Release of Information (ROI) requests in timely manner (as per WA state law).
- Processes and distributes the incoming mail.
- Works directly with third party payers in response to Healthcare Effectiveness Data and Information Set (HEDIS) requests, which is the governing agency that measures quality for the payors.
- Answers phone calls in a timely and professional manner; responds promptly and appropriately to requests.
- Works closely with providers to answer questions related to medical records.
- Maintains confidentiality, privacy and security of protected health information.
- Performs other related duties and special projects as assigned.

Required Education and Experience
- High school diploma or equivalent; associate’s degree preferred.
- One year of clerical experience; medical records/medical office experience preferred.
Required Knowledge, Skills, Abilities
- Able to thrive in a fast-paced and high volume professional environment while maintaining timely and accurate records.
- Works professionally as a member of the team and has interpersonal skills required to interact with staff members and patients.
- Proficient in the use of current technology, including Microsoft Office products, internet, copiers, scanners, telephones, and faxes.
- Knowledge of HIPAA regulations and requirements; ability to apply regulations and policies as required to ensure proper handling of confidential patient records and to explain the law and the privacy policy to patients.
- Demonstrate keyboarding and data entry skills and ability to enter data quickly and accurately.
- Strong organization abilities and attention to detail.
- Outstanding customer service skills.
- Professional and effective written and verbal communication skills.
- Ability to identify and employ communication strategies appropriate to the audience.
- Ability to develop processes and systems and to maintain accurate documentation and records.
