Role Summary: This position involves providing safe, effective, and cost-efficient pharmaceutical care, including developing and monitoring evidence-based pharmacotherapy plans to improve patient outcomes and quality of life. Key Responsibilities: Deliver comprehensive pharmaceutical care tailored to specific patient care areas.Interpret and clarify physicians' medication orders, review patient medication profiles for safety and interactions.Recommend formulary alternatives based on clinical parameters and consult with medical staff on drug therapy issues.Provide drug information, conduct patient discharge consultations, and make rounds with medical teams.Calculate dosages, verify orders, and prepare compounded or sterile medications.Supervise pharmacy technicians, students, and interns, ensuring accuracy and adherence to standards.Collaborate with healthcare professionals to resolve pharmacy-related issues and assist in policy updates. Qualifications & Skills: Bachelor's degree in pharmacy (Doctoral preferred), residency training or 3 years of clinical experience preferred. Strong communication, analytical, and supervisory skills are essential. Work Environment: Primarily clinical setting, focused on patient-centered care, safety, and collaboration within a multidisciplinary team.
