Hematology Pharmacist

Role Summary: The Pharmacist provides safe, effective, and cost-efficient medication therapy, developing evidence-based care plans focused on patient outcomes and quality of life. They assess medication appropriateness, monitor laboratory results, and ensure documentation accuracy. Key Responsibilities Deliver pharmaceutical care in designated patient care areas.Interpret and clarify physicians' medication orders.Review medication profiles for interactions, contraindications, and safety.Recommend formulary alternatives based on patient parameters.Advise healthcare professionals on drug therapy issues and monitor lab results.Provide drug information and patient discharge medication counseling.Collaborate with medical teams during rounds and calculate dosages as needed.Verify orders, prepare compounded and sterile medications, and manage supplies.Supervise pharmacy technicians, students, and interns, ensuring accuracy.Work with healthcare teams to resolve pharmacy service issues and update policies. Qualifications & Skills: A Bachelor's Degree in Pharmacy is required; a Doctoral Degree is preferred. Residency training or three years of clinical experience is desirable. Strong communication, collaboration, and clinical judgment skills are essential, along with a commitment to safety and quality care.
