Heart Failure RN Coordinator

Responsibilities

• Conducts new patient interviews and assesses patient's functional health and educational needs.
• Monitors patient's lab values, test results, and progress.
• Provides patient/family education.
• May assist in the physical exercise program, providing clinical oversight for individual patient progress and assessment of physical condition.
• Coordinates patient discharge needs to ensure smooth transition from hospital to outside agencies.
• Acts as a liaison between patient and other medical providers regarding patient progress, symptoms, and complications.
• Participates in clinical pathway development and utilization.
• Contacts insurance companies to verify coverage of services.
• Provides follow-up with patient after discharge to assess continuation of home program.
• Collaborates with multi-disciplinary teams for assessing and evaluating patient care.
• Participates in departmental continuing education and in-service programs.
• Works rotating schedules, which may include variable hours, weekends, nights, and holidays to meet staffing and patient care demands of a 24/7 complex health system.

Requirements

• Three years of general nursing experience.
• Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system (to be obtained within 30 days of hire).
• Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.

Qualifications

• Ability to perform essential functions of the job as outlined.
• Demonstrated availability to work variable and rotating shifts, including nights, weekends, and holidays, in a 24/7 patient care environment.
• Demonstrated human relation and effective communication skills.
• Ability to provide care appropriate to the population served.
• Ability to make nursing practice decisions in their area of specialty as defined by University of Utah Health policies and the Nurse Practice Act.

Preferred Qualifications

• Cardiology experience strongly preferred.
• ACLS card or obtain one within six months of hire.
• 3–5 years of nursing experience strongly preferred.
• Hybrid ability varies; must be able to come into the office regularly.

Working Conditions and Physical Demands

This position involves intensive work that may exert up to 100 pounds and may consistently require lifting, carrying, pushing, pulling, or otherwise moving objects, such as medical equipment or patients. Workers may be exposed to infectious diseases and may be required to function around prisoners or behavioral health patients.
