About the role

The Full Time Heart Failure Coordinator at St. Mary's Medical Center serves as the primary clinical nursing resource and central point-of-contact for the Heart Failure Program. This role involves coordinating and supporting nursing workflows, promoting exceptional patient care, efficient clinic operations, and adherence to organizational policies, evidence-based heart failure management practices, and management directives.

Responsibilities

• Provides coordination and support for nursing workflows
• Promotes exceptional patient care, efficient clinic operations, and adherence to organizational policies, evidence-based heart failure management practices, and management directives
• Facilitates effective communication, care coordination, and continuity of services among physicians, advanced practice providers, nursing staff, administrative personnel, patients, and external stakeholders
• Maintains a collaborative working relationship with and demonstrates effective followership to the Physician Operations Manager, supporting departmental goals, operational consistency, and the delivery of high-quality cardiovascular care across all Heart Failure Program clinic locations

Requirements

• Bachelor’s degree in Nursing or related field
• Current RN license in the state of employment
• Experience in a healthcare setting, preferably in cardiac care or similar specialty area
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite

Qualifications

• Advanced Cardiac Life Support (ACLS) certification
• Knowledge of evidence-based heart failure management practices
