Healthy Steps Specialist
About the role
The Healthy Steps Specialist at Evergreen Health and its affiliates is responsible for comprehensive coordination of medical care, linkage to supportive services, and retention in care to improve health outcomes for patients of Evergreen Health Primary and Specialty Care. They provide interventions, screenings, psychotherapy, and follow-up for families and pediatric patients from age 0 to 5.
Responsibilities
- Conducts team-based well child visits with parents and their babies and young children with the physician/pediatric nurse practitioner
- Provides consultations with families with babies and young children regarding developmental and behavioral concerns such as sleep, discipline, picky eating, etc.
- Collaborates with HS team to implement all core components of the program including universal child development and SDOH screening, making positive parenting and early learning guidance information readily available to all practice staff, and adjusting workflows for optimal efficiency.
- Provides consultation to medical professionals and all practice staff re: early childhood development, infant mental health, and trauma-informed practice.
- Provides referrals and tracks follow-up, as appropriate, to help families make connections to key resources within the community.
Requirements
- A Bachelor’s degree in early childhood education or related field; master’s degree in social work preferred
- Knowledge and experience in early childhood growth and development, parent-child relational health, infant and early childhood mental health, and/or family systems
- Excellent written and verbal communication skills
- Awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
Qualifications
- Early Childhood Education experience required; pediatric experience preferred

Pay
The actual rate of pay offered will be based on a variety of individualized factors, including but not limited to a candidate’s skills, relevant experience, educational background, internal equity, and market considerations. Higher side of range will be considered for candidate meeting highest level of degree, licensure, and experience.
