Member Duties
- Communicate with clinical staff to coordinate access to social determinant of health (SDOH) resources for patients
- Conduct annual PRAPARE screenings with patients after the clinical visit
- Update community resource guides to reflect all available SDOH resources within the BRMC service area
- Create and address telephone encounters (TEs) for patients in need of SDOH resources
- Conduct follow up calls to patients with identified SDOH needs to close loop of care for screened patients
- Document all PRAPARE screenings, results, and follow-up calls in the electronic health record (EHR)
- Assist patients with navigating both BRMC and community resources
- Advocate for patients in need of community resources to support holistic health of the patients
- Attend care management team meetings with patient care team
- Complete all trainings such as PRAPARE screening, motivational interviewing, EHR, HIPAA
