About the role
The Health Care Director (HCD) leads health and wellness activities for the community’s resident population and manages day-to-day healthcare operations. As the community clinical leader, the HCD oversees a team of nurses, medication technicians, and nursing assistants to ensure quality care and compliance with all applicable local, state, and federal regulations, as well as Harmony Senior Services policies and standards.
Responsibilities
- Evaluates residents prior to move-in to ensure their care and service needs can be met; conducts reevaluations as required by resident condition or state regulations.
- Develops and revises individualized service plans based on resident evaluations, preferences, and condition changes.
- Participates in interdisciplinary service plan reviews with residents, family members, physicians, community leadership, and third-party providers to ensure accurate reflection of care needs and resident safety.
- Collaborates with residents’ healthcare teams (physicians, pharmacists, etc.) to promote coordinated care.
- Manages the community’s medication management program, including audits, physician order oversight, controlled drug management, and compliance with regulations.
- Supervises and provides training for medication technicians, AMAP, and unlicensed personnel administering medications; conducts competency validations and observations per state regulations.
- Oversees C.N.A. and direct care personnel, ensuring resident needs are met through training and supervision.
- Delegates tasks appropriately within the scope of the nursing license held.
- Maintains up-to-date knowledge of regulations and ensures documentation and resident care meet compliance standards; prepares for surveys and participates in corrective action plans.
- Serves as the community’s infection preventionist, implementing measures to prevent the spread of infectious illness.
- Monitors resident care and quality indicators, addressing trends in weekly At-Risk or monthly QAPI meetings.
- Builds and develops the resident care team through hiring, onboarding, training, performance evaluation, coaching, and fostering a respectful culture.
- Collaborates with the Executive Director to operate the resident care department within budget.
- Works with the Director of Sales and Marketing on external business development opportunities.
- Performs other duties as assigned.
Requirements
- Current, valid, and unencumbered RN or LPN license in accordance with state regulations.
- Completion of state-required training courses for nursing in assisted living (e.g., Delegating Nurse course, state-specific medication training).
- Ability to apply critical thinking and nursing process principles to ensure safe, quality care.
- 1–2 years of experience required; 3+ years preferred.
- Supervisory/management experience strongly preferred.
- Current CPR certification.
- Ability to work occasional weekends, off-shifts, and take after-hours calls as needed.
- Physical ability to lift, carry, and push up to 50 pounds.
