Health Information Management Representative
About the role
The Health Information Representative is responsible for creating and reviewing Plans of Care to ensure CMS program compliance is met. This position handles technical reviews, audits, and maintenance of Home & Community Care patient records. They work collaboratively with clinicians and providers to ensure certifications, orders, and clinical documentation throughout the patients' care are completed and resolve any medical record issues impacting the revenue cycle process in a timely manner. This role ensures documents are scanned, indexed, and retained in a manner that safeguards the integrity of the medical record.
Responsibilities
- Create and review Plans of Care to meet CMS program compliance
- Handle technical reviews, audits, and maintenance of Home & Community Care patient records
- Collaborate with clinicians and providers to ensure certifications, orders, and clinical documentation are completed
- Resolve medical record issues impacting the revenue cycle process in a timely manner
- Ensure documents are scanned, indexed, and retained in a manner that safeguards the integrity of the medical record
Requirements
- 2 years experience in a team setting, including participation in a healthcare-related college program
- Six (6) months of the experience must be in a clerical or general office setting; clinical setting preferred
Qualifications
- Coursework/Training: Health Information Management, computer science, business, or related health care studies
- Coursework/Training: Medical Terminology
- OASIS Certification
- 6 months experience with an Electronic Medical Record (EMR) system
Skills
- Strong attention to detail
- Excellent communication skills
- Proficiency in Microsoft Office Suite
- Ability to work independently and as part of a team

Pay
The base pay range for this position is $17.26 - $25.87 per hour. Additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Schedule
This position is full-time and works on a day shift.
