What you'll do

The Health Coordinator supports Pioneer Human Services’ mission by coordinating and monitoring client physical and behavioral health services within residential treatment facilities. This role assists in ensuring continuity of care by facilitating communication between clients, nursing staff (where present), medical and behavioral health providers, pharmacy services, and program staff.

Client Service

  • Utilize program-approved screening tools and established procedures to identify and document client risks, needs, and barriers while supporting client and facility safety.
  • Utilize Motivational Interviewing (MI) and other engagement strategies to support client participation and skill development.
  • Interpret and explain program policies and goals to clients during orientation and throughout treatment as needed.
  • Support client education by conducting health education activities and reinforcing self-care skills appropriate to the client’s stage of recovery.
  • Observe and document client self-administration of medications in accordance with program policies and procedures.
  • Communicate medication-related concerns, discrepancies, or client requests to the licensed nurse.
  • Maintain a safe and healthy environment which includes maintaining, cleaning, and disposing of equipment and supplies according to policy and procedures (e.g., cleaning medication rooms, disinfecting vital signs equipment, maintaining health services spaces).
  • Assist with medication storage, ordering, and returns in accordance with pharmacy services policies and site procedures.
  • Support clients in meeting health and safety needs in accordance with each individual’s stage of recovery and program policies.
  • Provide basic first aid and serve as the first point of contact for emergency response when needed, notifying licensed nursing staff, providers, or emergency services as appropriate.
  • Follow provider orders and support implementation of care plans in accordance with program policy and the scope of the Health Coordinator role.
  • Gather and relay client-reported health information, including health and medication history, vital signs, symptoms, diagnoses, and other relevant concerns to nursing staff or providers as appropriate.
  • At sites with licensed nurses, assist RNs or LPNs with intake processes or coordination activities as directed.
  • Care coordination: Support coordination of client physical and behavioral healthcare services during assigned shifts, including telehealth visits, clinic appointments, and outside provider services.
  • Coordinate and document client intake and discharge processes after hours, including financial documentation and secured belongings as required.
  • Support service navigation by assisting clients in accessing health services and communicating needs to clinical staff.
  • File and document management: Maintain accurate, timely, and complete documentation of services in the electronic health record. Ensure required notifications are completed during assigned shifts. Complete eligibility assessments and follow-up assessments in designated systems. Ensure client information is accurate and updated from intake through discharge to support continuity of care. Record routine case notes and service contacts documenting client progress and services provided. Review reports with the care team to identify service gaps and coordinate appropriate follow-up support.

Qualifications

  • A High School Diploma or Equivalent.
  • One (1) year of experience working in a caregiving, human services, behavioral health, or social services setting, or experience supporting marginalized or vulnerable populations.
  • Must apply for Agency Affiliated Counselor registration within 30 days of hire and obtain and maintain active registration within 90 days of hire in accordance with WAC 246-810-015; RCW 18.19.210.
  • First Aid/CPR certification within 90 days of hire.
  • Proficient level of written/verbal communication, and efficiency with independent task management and ability to show attention to detail.
  • Basic computer skills and the ability to learn.
  • Demonstrated ability to maintain a professional, empathetic, and client-centered approach when working with individuals in recovery.
