About the role
The Health Coach is a non-clinical member of the Heart Failure care team, supporting patients through education, behavior change, and engagement within Novocardia’s value-based care model. This role partners closely with clinical team members to empower patients to better understand and manage their heart failure through prevention-focused, patient-centered coaching.
Key responsibilities
- Patient Education & Engagement
- Provide education and coaching related to heart failure, lifestyle modification, medication adherence, nutrition, physical activity, and self-management.
- Establish short- and long-term goals with patients using shared decision-making and motivational interviewing techniques.
- Encourage sustained patient engagement, self-advocacy, and behavior change throughout program participation.
- Care Team Collaboration
- Partner with the clinical care team to support patient care plans and escalate concerns appropriately.
- Provide timely updates and documentation related to patient engagement and progress.
- Participate in interdisciplinary team meetings.
- Respond to patient communications in a timely, professional, and compassionate manner.
- Build and maintain strong, trust-based relationships with patients and caregivers.
- Consistently deliver a high-quality patient experience aligned with program standards.
Qualifications
- Education & Experience
- Bachelor’s degree in health sciences or related field preferred.
- NBC-HWC (National Board for Health & Wellness Coaching) certification required.
- Experience in a healthcare, chronic disease management, or patient engagement setting preferred.
- Cardiology or heart failure experience a plus.
- Telehealth or remote patient engagement experience preferred.
- Skills & Competencies
- Strong communication and interpersonal skills.
- Ability to motivate and support behavior change in diverse patient populations.
- Organized, adaptable, and comfortable managing multiple priorities.
- Profiency with electronic health records and standard computer applications.
- Bilingual English/Spanish proficiency preferred.
- Work Environment
- Remote position requiring a dedicated, private workspace and reliable high-speed internet to ensure patient privacy and compliance with HIPAA standards.
