Health Coach

About the role

The company partners with health systems, community health centers, and independent specialists to remove barriers to care. The health coach will play a key role in helping patients reverse chronic disease by using specialized education in diet and nutrition, working alongside physicians, nurse practitioners, and the Patient Success team to create personalized care plans.

Responsibilities

• Utilize evidence‑based health coaching interventions to improve patient engagement and outcomes.
• Guide patients through healthcare‑provider‑created treatment plans and partner with providers to deliver safe, effective coaching and care.
• Collaborate with the Care Team to develop integrated treatment plans and partner with medical providers to ensure safe, effective treatment and coaching.
• Provide AndHealth integrated health coaching to a full patient panel, targeting sleep, stress, nutrition, movement, and medication adherence under the guidance of practitioner‑designed care plans.
• Deliver behavior‑change coaching using stages of change to a full patient panel.
• Collect, interpret, and track patient‑reported outcomes and wearable data, using this information to inform individualized treatment plans in collaboration with the patient’s medical provider.
• Support patients with personalized, medically necessary nutrition plans under the direction of a registered dietitian to reduce severity and frequency of migraine attacks and/or autoimmune flares.
• Conduct virtual, synchronous treatment sessions to evaluate progress and adapt interventions to patient needs.
• Engage patients in asynchronous, individualized coaching as needed via technology platforms.
• Follow clinical workflows and processes using an EHR and other key tools to document patient outreach and progress effectively.
• Actively contribute to process improvements by providing regular feedback to internal stakeholders.
• Assist with special projects as needed.

Requirements

• Bachelor’s degree is preferred.
• Completion of a health coach training program that qualifies you to sit for the NBHWC Certification exam preferred.

Qualifications

• Established experience working with patients with chronic diseases.
• Demonstrated experience working within a medical model and/or an interdisciplinary team.
• Experience and comfort working with patient data, including utilizing and adapting to various technology platforms.
