Health Center LPN / RN

About the role

The Health Center Nurse (LPN/RN) provides compassionate, patient-centered care within a primary care team, working under the supervision of providers and the Practice Manager. This role involves performing diverse clinical tasks, collaborating with healthcare teams, and supporting community health efforts to improve patient outcomes.

Responsibilities

Key responsibilities include patient assessment, care coordination, medication administration, health education, chronic disease management, preventative care, and assisting with clinical procedures such as dressings, vaccinations, catheterization, EKGs, and lab tests.

• Conduct patient intake, vital signs, and health history interviews.
• Administer medications, vaccines, allergy injections, and perform procedures like suturing and catheterization.
• Perform screenings including vision, audiometry, and EKGs.
• Provide health education on treatment plans and participate in community health initiatives.
• Draw blood, perform lab testing, and record results in electronic medical records.
• Dispense pharmaceuticals, monitor inventory, and maintain accurate documentation.
• Ensure infection control through sterilization and office protocols.
• Participate in quality assurance activities and protocol development.

Qualifications

• Graduate of accredited LPN or RN program; licensed in Vermont.
• Provider-level CPR/BLS certification.
• Minimum one year of experience in a community health or medical setting.
• Knowledge of disease processes, medications, and public health issues.
• Effective communication, teamwork, multitasking, and problem-solving skills.
• Proficiency with EMRs, basic lab tests, and computer tools.
• Physical ability to lift up to 50 pounds, stand for extended periods, and work under stress.
• Adherence to confidentiality and safety protocols, with exposure to bodily fluids expected.
