About the role

Under the general supervision of an RN and/or Physician, the Health Care Coordinator supports providers in delivering healthcare, including recording vital signs, discussing chief health concerns, performing testing, and assisting with procedures and documentation for designated approved OASAS programs. You will assess, plan, evaluate, and deliver patient care in collaboration with the RN and/or Physician. As a member of the interdisciplinary clinical team, you will participate in case conferences, treatment, and discharge planning meetings. The role also involves serving as a liaison between the agency’s clinical treatment team and external primary medical care providers to ensure continuity of care for clients with medical conditions.

Responsibilities

  • Assist RN or Physician with patient care, including checking vital signs (blood pressure, pulse rate, breathing, temperature, and weight), discussing chief health concerns, and performing tests.
  • Take patient’s medical history, complete pretreatment formalities, and enter data into the computer system.
  • Follow orders of physician and registered nurse in providing care.
  • Ensure medications are administered to the intended patient safely and correctly, with proper documentation.
  • Assist self-directed patients in taking medications.
  • Handle proper charting, documentation, and maintenance of medical records.
  • Record detailed reports of care given in patient clinical records.
  • Reinforce instructions given to patients by RN or Physician.
  • Notify RN or Physician of significant findings or changes in patient’s physical or mental condition.

Requirements

  • BLS certification.
  • High School Diploma or Equivalent.
  • At least one (1) year of experience working in a human services program in a caregiving role.
  • Available to work a flexible schedule as needed in response to participant and staff needs.
  • Computer literacy, including proficiency in Microsoft Office Suite and EHR.
  • Ability to put patients at ease and explain physician’s instructions.
  • Strong multi-tasking and organizational skills.
  • Ability to maintain the confidentiality of patient records.
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
  • Knowledge of medical terminology.
  • Team player with the ability to work independently.

Pay

$26.37–$32.96 per hour

Schedule

Tuesday–Saturday, 2:00 PM–10:00 PM
