About the role
The Health and Social Care Coordinator (HSCC) plays an integral role within the Caithness Integrated Team, gathering referral details from various sources including clients, their family members, and other Health and Social Care Professionals. This information is then shared with the multi-disciplinary team in order to prioritise and allocate. The HSCC provides support to professional staff when dealing with urgent and critical cases in the community.
