Pay
$21.50 per hour weekdays, $24.00 per hour weekends. Starting pay is $20/hr, exceeding the state average. Opportunities for extra pay based on shifts worked.

About the Role
Provides personal care, light housekeeping, and assistance with Activities of Daily Living (ADLs) as outlined in each participant’s plan of care in their homes and at the PACE center. Exercises independent judgment, reports changes in participant status to the Interdisciplinary Team (IDT), and participates in infection control and cleaning protocols per CDC guidelines.
Responsibilities
- Assists with ADLs for participants at the PACE center and in their homes (community, Assisted Living Facilities, Rest Homes, Supportive Housing programs, etc.) under RN supervision.
- Contributes to participant care plan development through collaboration with the Interdisciplinary Team.
- Performs non-skilled treatments (e.g., vital signs, transfers, toileting, bathing, dressing) at the PACE Center and in the community.
- Assists with recreational activities, including planning, motivating participation, and evaluating program effectiveness.
- Supports registered therapists and certified therapy assistants with participant-specific activities (e.g., positioning, transfers, ambulation, exercises).
- Maintains a clean and safe environment at the PACE center and participants’ homes.
- Assists with meal/snack preparation, serving, feeding, and clean-up.
- Uses safe techniques in all participant interactions.
- Documents accurately and timely in the EMR and other systems as required.
- Participates in daily cleaning and disinfecting of the PACE center per CDC guidelines.
- Actively distributes work assignments to ensure timely completion of participant care and site tasks.
- Participates in regular team meetings for health aides.
- Performs duties in accordance with Fallon Health and Summit ElderCare policies.
Requirements
- Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program (or equivalent work experience).
- Certification as a Home Health Aide or Nursing Assistant is desired.
- Reliable transportation required.
- One year of experience working with frail and/or older adults (preferably in a PACE program). If under one year, must complete Summit ElderCare’s training program before working independently.
- Must complete annual standardized competencies for Health Aides.
