Harm Reduction Case Manager

About the role

Join us in our mission as a Harm Reduction Case Manager at Care Coordination Services. You will play a pivotal role in individuals' lives, making a significant impact through outreach activities, harm reduction strategies, and client linkage.

Responsibilities

• Conduct outreach activities to engage the target population
• Identify community partners to engage PWUD who are unaware of their status or untreated
• Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks
• Provide enrollment into the agency’s Syringe Exchange Program
• Facilitate brief assessments with the target populations
• Review of the assessment to identify community-based services to provide client linkage
• Facilitate linkage to services through escort as necessary

Qualifications

• A High School Diploma or GED
• 1-3 years of relevant experience in Human Services
• A valid Driver's License and access to a reliable vehicle

Pay

Minimum Salary: $42,000
Maximum Salary: $42,000

Schedule

Mon – Fri 8:30am – 4:30pm
