Habilitation Support Professional

About the Role

The Habilitation Support Professional (HSP) supports individuals with Intellectual and Developmental Disabilities in the Home and Community Based Setting (HCBS) and the Day Habilitation programs. The HSP follows the Individualized Service Plan (ISP) and/or the Day Habilitation Plan, promoting independence and person-centered decision making.

Responsibilities

• Ensure safety of clients in home or day habilitation settings while promoting independence and person-centered care.
• Assist with personal hygiene needs of clients.
• Collaborate with clients, guardians/advocates, and supervisors to identify valued outcomes for individualized habilitation plans.
• Implement valued outcomes daily using agency documentation procedures.
• Develop and implement activities with clients and their primary caregivers.
• Accompany clients on trips and outings, supervising activities.
• Participate in interdisciplinary team meetings to review progress and plan future outcomes.
• Document client experiences and observations in the electronic health record, including reporting any medical or behavioral incidents to supervisors.
• Follow all safeguards listed in individuals’ treatment plans.
• Dispense medications as needed (specific to Day Habilitation).
• Complete OPWDD documentation requirements, including monthly checklists and summaries.
• Adhere to agency timekeeping guidelines via Dayforce and complete mandatory trainings.
• Complete ongoing OPWDD and other required trainings.
• Perform physical tasks such as lifting clients (using mechanical devices), positioning, pushing wheelchairs, and transferring clients as prescribed.

Requirements

• High School diploma or a minimum of 1 year of experience working with the IDD population.
• Must be at least 18 years of age.
• AMAP certification required for Day Habilitation setting during probationary period.
• Experience working with individuals with Intellectual Disabilities preferred.
• Basic computer skills, including proficiency with agency software programs and Electronic Health Record.
• Driver’s License preferred to meet insurance requirements.
• Ability to review treatment plans and document daily logs.
• Manual dexterity to work with clients on various activities.
