Gyn Surg Oncology Nurse Care Coordinator

Responsibilities

• Affirms and communicates patient and family care needs throughout the patient’s stay.
• Serves as the primary coordinator and point of contact for the patient care plan.
• Provides clinical guidance and support to bedside staff for managing complex patient needs and assisting with complex procedures.
• Collaborates with other Care Coordinators and Charge Nurses to assess and manage the workload of the interdisciplinary team.
• Attends medical rounds at least two mornings per week.
• Participates in patient evaluation conferences twice weekly, updating on patient progress, discharge barriers, and other concerns.
• Attends and participates in team program meetings.
• Monitors and updates nursing orders as needed based on care plan changes.
• Attends discharge briefings to discuss discharge barriers and needs.
• Makes post-discharge follow-up phone calls within one week of discharge.
• Works rotating schedules, including variable hours, weekends, nights, and holidays, as needed to meet operational and patient care demands.

Requirements

• Bachelor's Degree in Health Care Administration, Health Science, or related field, or equivalent.
• One year of related nursing experience.
• Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system.
• Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah.

• Excellent human relations and effective communication skills.
• Demonstrated availability to work variable and rotating shifts, including nights, weekends, and holidays, in a 24/7 patient care environment.
