Group Facilitator

This is a hybrid role that combines the responsibilities of a Group Facilitator and Case Manager. The individual provides therapeutic group facilitation while also supporting clients through case management services and coordination of care.

Responsibilities

• Provides direct support services, including engaging clients in Group Therapy, topics related to 12-step recovery, health and wellness promotion, and crisis management.
• Conducts assessments or screenings with clients to evaluate their presenting problems, determine needs, and develop appropriate treatment plan goals.
• Develops and provides individual treatment plans; monitors, evaluates, and documents client progress.
• Handles intake documentation and prepares progress notes, event notifications, and other client-related documentation in accordance with regulatory requirements and company policies.
• Monitors case load daily to ensure required documentation (assessments, treatment plans, case reviews, case management sessions, discharge plans) is completed timely.
• Facilitates coordination of care, including case review, therapeutic interventions, and client progress in treatment.
• Assists and coaches recovering individuals in developing their own recovery plans.
• Facilitates transition from professional services to community-based services.
• Promotes self-advocacy by ensuring clients' voices, needs, goals, and objectives are the focal point of rehabilitation and clinical services.
• Identifies and supports linkages to community resources (recovery communities, educational, vocational, social, cultural, spiritual resources, mutual self-help groups, professional services).
• Provides hope and encouragement for opportunities at varying levels of community involvement (work, school, relationships, physical activity, self-directed hobbies).
• Develops relationships with community groups/agencies in partnership with others in the agency.
• Identifies barriers (internal and external) to full participation in community resources and develops strategies to overcome them.
• Provides services via video conferencing/teletherapy when appropriate, including evaluation, assessment, consultation, treatment planning, case management, coaching, individual therapy, and group therapy.
• Completes other tasks as assigned.

Requirements

• Bachelor’s Degree in a non-related field and at least one year of full-time Behavioral Health work experience, or
• Associate degree and at least two years of full-time Behavioral Health work experience, or
• High School Diploma or equivalent and at least three years of full-time Behavioral Health work experience.
• Knowledge of software programs for personal computer use, including word processing, video conferencing, email, and spreadsheets.
• Experience in counseling and case management with substance abuse clients.
• Ability to provide services via teletherapy (interactive audio, video, telephone, and/or other audio/video communications).
• Ability to prepare, implement, and analyze treatment plans and psychiatric reports.
• Strong problem-solving skills and ability to carry out assigned projects to completion.
• Strong interpersonal skills, including effective verbal and written communication, relationship-building, and conflict resolution.
• Ability to plan, organize case workload, and manage deadlines.
• Commitment to maintaining ethical professional standards, including client confidentiality and boundaries.
• Demonstrated respect for cultural differences and diversity, and protection of client rights.
• Recognition of professional expertise limits and willingness to seek supervision/support as needed.
• Ability to make clinical and organizational decisions with clients' and agency’s well-being in mind.
• Provides treatment promoting client dignity, independence, individuality, strength, privacy, and choice.
• Recognizes symptoms of mental disorders, personality disorders, or substance abuse.
• Identifies types of medications commonly prescribed for mental disorders, personality disorders, and substance abuse, including side effects and adverse reactions.
• Provides ethical and professional services ensuring compliance with confidentiality laws (e.g., HIPAA).
• Recognizes, prevents, and responds to situations where a client may be a danger to themselves or others, behaves aggressively, or experiences a crisis/medical emergency.
• Recognizes and responds to fire, disaster, hazard, and medical emergencies.
• Completion of all training requirements and ability to pass background checks.

Physical Demands

• Ability to walk, bend, stand, climb stairs, speak English, and hear.
• Ability to use a telephone, drive an automobile, and operate standard office equipment (computer, copier, fax machine).
• Ability to lift and carry up to 25 pounds.
