Graduate Practical Nurse - Home Care Academy - Private Duty

About the role

Vanderbilt Health is seeking a Private Duty Nurse to provide direct care to patients in the home setting. This role is part of the Home Care Academy Program, designed for recently graduated LPNs.

Responsibilities

• Collaborates with patients/families and healthcare providers to deliver health care services
• Supports the healthcare team by following the established plan of care
• Navigates intense medical scenarios with autonomy, under the direction of the supervising RN
• Administers intravenous fluids and performs sterile procedures
• Recognizes and responds to clinical situations appropriately and timely to prevent patient decompensation and injury
• Identifies and assesses patient needs and clinical changes, accurately reporting findings to the supervising RN and physician

Requirements

• Graduate of an approved discipline-specific program
• LIC - Licensed Practical Nurse in Tennessee
• Less than 1 year of relevant work experience

• Intermediate Clinical Competency
• Intermediate LPN Patient Education
• Intermediate LPN Care Planning
• Intermediate Clinical Patient Documentation
