About the role
Vanderbilt Health is seeking a Private Duty Nurse to provide direct care to patients in the home setting. This role is part of the Home Care Academy Program, designed for recently graduated LPNs.
Responsibilities
- Collaborates with patients/families and healthcare providers to deliver health care services
- Supports the healthcare team by following the established plan of care
- Navigates intense medical scenarios with autonomy, under the direction of the supervising RN
- Administers intravenous fluids and performs sterile procedures
- Recognizes and responds to clinical situations appropriately and timely to prevent patient decompensation and injury
- Identifies and assesses patient needs and clinical changes, accurately reporting findings to the supervising RN and physician
Requirements
- Graduate of an approved discipline-specific program
- LIC - Licensed Practical Nurse in Tennessee
- Less than 1 year of relevant work experience
Qualifications
- Intermediate Clinical Competency
- Intermediate LPN Patient Education
- Intermediate LPN Care Planning
- Intermediate Clinical Patient Documentation
Skills
- Excellent communication skills
- Ability to handle multiple assignments simultaneously
- Strong problem-solving skills
