Position Responsibilities

  • Performs medical examinations, evaluations, assessments, diagnoses and treatment to long term chronic care patients post-acute care inpatients, and/or outpatients
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions
  • Knowledge of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosages and instruct patients in correct usage
  • Refers patients to specialists and to relevant patient care components as appropriate
  • Directs and coordinates the patient care activities of clinical team members and support staff as required
  • Captures each patient encounter in a timely manner (typically the same day) using the appropriate electronic medical record system
  • Signs/completes documentation in a timely manner to allow for submission of charges by off-site billing company

Qualifications

  • Education: Awarded degree of MD or DO with ABIM recognized internal medicine or family practice residency (3 years minimum). Successful completion of ACGME accredited geriatric medicine fellowship program preferred
  • Licenses: Valid and unrestricted Massachusetts Medical License. Certification American Board Certification in Internal Medicine or Family Medicine currently valid or renewed (if needed) prior to expiration. ABIM added certification in geriatric medicine preferred. Federal DEA Certification
  • Certifications: CPR, BLS, ACLS as indicated by specific practice location
  • Experience: Experience, gained in training and/or in practice, caring for seniors in long term chronic care, community or home settings. Experience delivering effective medical care within an inpatient environment
  • Knowledge: Ability to maintain quality, safety, and/or infection control standards. Knowledge of community medical diagnostic and patient care services in area of medical expertise
  • Skills: Experience working both independently and in a team environment. Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise. Ability to supervise, advise and train clinical professionals and/or students in area of expertise
