Geriatric Nurse Practitioner or Physician Assistant - PACE

Responsibilities

• Complete all required documentation in accordance with department standards
• Work effectively with patients from diverse social, cultural, and economic backgrounds
• Participate in PACE Interdisciplinary Team (IDT) meetings
• Maintain all necessary certifications and licensures required
• Attend all mandatory meetings; minimize incident reports; use resources efficiently and practice medicine in a cost-effective manner
• Maintain FHCSD behavioral standards, including effective listening and communication skills and respectful interactions with colleagues and patients
• Able to assess medical information including pathophysiology, patient presentation, differential diagnosis, etc., to facilitate health promotion and disease prevention
• Maintain satisfactory peer reviews; prioritize work and adhere to schedule
• Participate in Clinical Outcomes Program and work towards meeting organizational targets for all metrics
• Participate in administrative duties, including lab reports, charts, and medication refills reviewed and managed appropriately
• Perform other duties as assigned

Requirements

• Minimum of 2 years of experience working with underserved, low-income older adult persons in an outpatient clinic or similar setting required
• Completion of an approved/accredited Physician Assistant or Nurse Practitioner program
• Current unrestricted license to practice in the State of California or ability to obtain DEA license
• Ability to obtain American Heart Association health care provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training
• Meet FHCSD Driving Requirements
• Second language fluency desired

Qualifications

The ideal candidate must have a minimum of 2 years of experience working with underserved, low-income older adult persons in an outpatient clinic or similar setting. Completion of an approved/accredited Physician Assistant or Nurse Practitioner program is required, along with a current unrestricted license to practice in the State of California or ability to obtain a DEA license. Ability to obtain American Heart Association health care provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training is also required. Meet FHCSD Driving Requirements and second language fluency is desired.
