Geriatric Nurse Practitioner - Locum Tenens Coverage

About the role

We are seeking a Geriatric Nurse Practitioner for clinic coverage, providing primary care services to the elderly population in a structured outpatient setting.

Responsibilities

• Managing chronic conditions such as dementia, arthritis, diabetes, and cardiovascular diseases
• Conducting physical exams and cognitive screenings
• Oversight of medication management to ensure safe prescribing and minimize polypharmacy
• Coordination of patient care transitions between hospitals, rehab centers, and home care
• Supporting patient care with the assistance of MA and RN staff

Requirements

• Board certification
• Prescriptive authority
• No call duties
• Day shift in an on-site clinic
• Serving a geriatric population

Qualifications

• Master's degree in Nursing
• Nurse Practitioner certification
• Experience in geriatric care
• Knowledge of pharmacology and medication management
