Requirements

• Active and unrestricted license in the state of Illinois
• Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) degree
• Willingness to become credentialed with various dental insurance plans
• Physical stamina to lift and carry up to 45 pounds
