Qualifications
- Doctor of Dental Surgery (DDS) or Doctor of Medicine in Dentistry (DMD) from an accredited institution or current attendance at such a program
- Valid dental license or eligibility to obtain one
- DEA certification
- CPR/BLS certification or ability to obtain
