About the Role

Provide and coordinate nursing care for patients in their homes, developing individualized care plans and ensuring compliance with healthcare regulations.

Responsibilities

• Develop, implement, and revise individualized care plans in collaboration with healthcare teams.
• Supervise and instruct home health aides and nurses, ensuring compliance with regulations.
• Conduct comprehensive patient assessments, document visits accurately, and complete OASIS data sets.
• Evaluate patient eligibility and communicate effectively with patients, families, and physicians to ensure quality care.
• Participate in quality improvement activities, team orientation, and performance improvement initiatives.
• Support survey readiness and patient safety initiatives.
• Work comfortably in diverse home environments.

Requirements

• Unrestricted RN licensure in the practice state.
• Valid driver’s license and reliable transportation.
• CPR certification.

• Strong communication, organizational, and writing skills.
