Full-Time Program Coordinator

Responsibilities

- Provides continuous supervision to case management staff and oversees the completion of performance evaluations - Conducts community outreach and engagement, including following up on referrals and proactively connecting with individuals in the community who may benefit from services - Completes initial screenings and intake assessments to determine eligibility and facilitate admission into the program - Develops service plans, generates monthly reports and statistics - Maintains an accurate record of consumer admissions, discharges, transfers, and hospitalizations - Collaborates with community providers and partner agencies to coordinate services, share information, and support continuity of care.

Qualifications

- MSW in Social Work or MS/MA in Psychology or a related field - Two years of post masters experience - Previous supervisory experience preferred - Valid Driver’s license
