About the Role
The Physician will provide comprehensive medical evaluation and management for patients in post-acute settings, including skilled nursing and rehabilitation facilities.
- Leading and coordinating post-acute medical care plans
- Managing acute and chronic conditions during recovery
- Providing symptom management and goals-of-care discussions
- Collaborating with interdisciplinary teams, including nursing, therapy, and case management
- Supporting safe transitions of care and reducing readmissions

Pay
The starting salary for this position is $309,000 annually. The actual compensation package may vary based on factors such as experience, internal equity, and alignment with market data.
