About the Role
A Physical Therapist (PT) provides in-home therapy services to help patients recover from illness or injury, supporting mobility, strength, and function.
Responsibilities
- Conduct initial and ongoing assessments to evaluate patient capabilities.
- Administer tests, interpret results, and set functional goals.
- Develop and update personalized plans of care, documenting progress and modifying as needed.
- Deliver therapy services in accordance with physician orders and ensure proper discharge planning.
- Collaborate with healthcare team members, patients, and caregivers, providing education and support.
- Supervise other personnel (PTA, HHA) where applicable and complete assessments per regulations.
- Maintain compliance with policies, procedures, and legal standards, ensuring accurate documentation to avoid claim denials.
Requirements
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license.
- Proficiency in assessments, documentation, and compliance.
Preferred Qualifications
- Experience in home health therapy and OASIS documentation.
