About the role
Provide prescribed physical-therapy treatments that involve complex, sophisticated modalities designed to restore clients to their maximum rehabilitation potential. This position travels throughout Montgomery County.
Responsibilities
- Perform initial and ongoing physical-therapy assessments to determine client functional level, including designated OASIS items for coordination of care among other disciplines.
- Perform direct therapeutic physical-therapy services as planned by the Agency staff and certified by the client’s physician.
- Evaluate equipment needs to increase functional level of clients.
- Assess, report, and record client’s status and progress to the physician and Agency staff members.
- Perform interim evaluation, if necessary, for recertification.
- Evaluate the effectiveness and outcomes of care.
- Teach and supervise nurses, home-health aides, caregivers, and clients to provide a continuous physical-therapy program.
- Consult with other Agency staff regarding goal setting and desired outcomes.
- Participate in clinical conferences and/or in-service programs to share with Agency staff in meeting rehabilitation goals.
- Identify barriers to desired outcomes.
- Deliver services to clients in accordance with accepted standards of treatment specifically effective for client conditions.
- Assist with the development, writing, and revision of the plan of care in collaboration with Agency staff and the physician.
- Consult with the Nurse Case Manager, Director of Professional Services, VP of Clinical Services, or designee regarding discharge planning with the client and caregiver.
- Notify the Agency of imminent discharge so proper notification can be mailed to the physician.
- Maintain appropriate clinical records in a prompt and comprehensive manner, documenting client limitations and progress.
- Develop and instruct Agency staff and caregivers in maintenance-therapy programs as approved by the physician.
- Assume responsibility for equipment signed out for client use.
- Evaluate designated OASIS items upon discharge from services for coordination of care among other disciplines.
Requirements
- Current Physical Therapy License in Maryland.
- Graduation from an approved physical-therapy program.
- One year of clinical experience.
- Current CPR certification.
- Excellent communication, organizational, and human-relations skills.
- Able to function with minimal supervision.
- Able to manage multiple projects and changing priorities.
- Able to employ critical-thinking skills.
