Responsibilities

  • Affirms and supports the YNHHS Vision, Mission, and Values.
  • Affectionately assesses and monitors the physical and psychological state of patients continuously.
  • Develops, communicates, and documents a personalized care plan that aligns with patient goals and involves the patient and healthcare team.
  • Enforces and collaborates on the implementation of the care plan, evaluating its effectiveness and making necessary adjustments.
  • Plans and delivers educational sessions tailored to the individual learning needs and readiness of patients and their families.
  • Assists in training new staff members.
  • Manages and supervises the activities of non-RN personnel on the assigned unit.
  • May be required to work on other nursing units based on staffing and patient needs.
  • Ensures a safe environment for both the RN and patients.
  • Meets performance standards in areas such as Customer Service, Teamwork, Resource Utilization, and Staff and Self Development.
  • Performs additional duties as assigned to maintain operational efficiency.
  • Specific Unit Responsibilities:
  • Exhibits proficiency in lung assessment and instructs patients on proper breathing techniques.

Qualifications

  • Graduated from an accredited nursing program; a Bachelor of Science in Nursing (BSN) is preferred.
  • Proficient in using computer software, particularly electronic patient records.
  • Ability to conduct internet research for patient care literature.
  • No prior experience required.
  • Valid State of Connecticut Registered Nurse license.
  • BLS certification is mandatory, with ACLS certification required within six months.
