Responsibilities
- Affirms and supports the YNHHS Vision, Mission, and Values.
- Affectionately assesses and monitors the physical and psychological state of patients continuously.
- Develops, communicates, and documents a personalized care plan that aligns with patient goals and involves the patient and healthcare team.
- Enforces and collaborates on the implementation of the care plan, evaluating its effectiveness and making necessary adjustments.
- Plans and delivers educational sessions tailored to the individual learning needs and readiness of patients and their families.
- Assists in training new staff members.
- Manages and supervises the activities of non-RN personnel on the assigned unit.
- May be required to work on other nursing units based on staffing and patient needs.
- Ensures a safe environment for both the RN and patients.
- Meets performance standards in areas such as Customer Service, Teamwork, Resource Utilization, and Staff and Self Development.
- Performs additional duties as assigned to maintain operational efficiency.
- Specific Unit Responsibilities:
- Exhibits proficiency in lung assessment and instructs patients on proper breathing techniques.
Qualifications
- Graduated from an accredited nursing program; a Bachelor of Science in Nursing (BSN) is preferred.
- Proficient in using computer software, particularly electronic patient records.
- Ability to conduct internet research for patient care literature.
- No prior experience required.
- Valid State of Connecticut Registered Nurse license.
- BLS certification is mandatory, with ACLS certification required within six months.
