Schedule
Full‑Time. Shifts: AM 6 am – 2 pm, Evening 2 pm – 10 pm, Night 10 pm – 6 am. Each shift is part‑time 32 hours/week with pick‑up opportunities.
Pay
$16.00 – $21.00 per hour, based on experience.

Responsibilities
- Assist residents with activities of daily living (ADLs) as indicated on the individualized service agreement.
- Help each resident reach and maintain the highest level of performance and independence.
- Provide physical and emotional support to residents.
- Assist with treatments delegated by the Registered Nurse per the resident service agreement.
- Observe clients and report changes in physical or emotional condition (e.g., attitude, appetite, weight) to the case manager.
- Interact with residents to provide reassurance, engagement, and redirection as appropriate.
- Complete all required documentation daily, including service schedule, flow sheet, communication book, etc.
Qualifications
- WI CBRF Certification (preferred).
- Six months of experience in long‑term care or assisted‑living residence (preferred).
- Registered as a Nursing Assistant, Home Health Aid, or Certified Nursing Assistant (preferred).
- Ability to make responsible choices and decisions and act in the resident’s best interests.
