About the role
We are seeking a Full-Time Licensed Practical Nurse (LPN) to provide home-health care for approximately 40 patients weekly, focusing on hypertension and diabetic management.
Responsibilities
- Conduct home visits
- Perform patient assessments
- Monitor vitals
- Provide medication education
- Accurately document each visit
- Communicate effectively with the RN Case Manager about patient changes
- Travel between locations (mileage reimbursement provided)
Requirements
- Active LPN license in Massachusetts
- Home-health experience preferred
- Strong communication and patient-education skills
- Reliable, organized, and able to work independently
