Full Time LPN

About the role

We are seeking a Full-Time Licensed Practical Nurse (LPN) to provide home-health care for approximately 40 patients weekly, focusing on hypertension and diabetic management.

Responsibilities

• Conduct home visits
• Perform patient assessments
• Monitor vitals
• Provide medication education
• Accurately document each visit
• Communicate effectively with the RN Case Manager about patient changes
• Travel between locations (mileage reimbursement provided)

Requirements

• Active LPN license in Massachusetts
• Home-health experience preferred
• Strong communication and patient-education skills
• Reliable, organized, and able to work independently
