Full-Time Licensed Practical Nurse

Position Overview The Licensed Practical Nurse plays a vital role in providing skilled nursing care tailored to individual patient needs, following care plans and physicians’ orders while maintaining confidentiality and professionalism. Key Responsibilities Deliver quality patient care in accordance with physician directives under RN supervision.Perform hands-on care, document activities, and communicate promptly with physicians and caregivers.Identify patient condition changes, implement appropriate interventions, and mitigate risks.Educate patients and families based on specific health needs.Provide home-based care using skills such as phone triage, wound management, infusions, catheter care, and dressing changes. Qualifications & Skills Active LPN license in the applicable stateValid CPR certification and a driver’s licensePreferred: Medicare Skilled Nursing experience, familiarity with Oasis, and at least one year of home health or healthcare experience; Home Care Home Base experience is a plus.
