Full time Hospice RN Case Manager

Responsibilities

• Assess patients' health care needs and develop individualized care plans
• Coordinate patient care with physicians, family members, and other healthcare professionals
• Ensure compliance with state and federal regulatory requirements in hospice care
• Monitor patients' symptoms, medication responses, and overall progress
• Provide education and emotional support to patients and their families
• Facilitate and attend interdisciplinary team meetings to discuss patient care
• Document patient care accurately and in a timely manner in accordance with company policies

Requirements

• Valid AZ RN license in the state of practice
• Minimum of two years of nursing experience, preferably in hospice or palliative care
• Strong assessment and interpersonal skills to effectively communicate with patients and families
• Ability to work independently and as part of a collaborative team
• Career-focused and committed to hospice care
• Compassionate and empathetic approach towards patient care
• Knowledge of hospice regulations and best practices
• Basic computer skills for documentation and communication purposes
