Responsibilities: Provide skilled nursing care to patients in their homes across the Northern Virginia areaConduct comprehensive patient assessments and develop individualized care plansAdminister medications, wound care, IV therapy, and other clinical treatments as orderedMonitor and document patient progress, vital signs, and changes in conditionEducate patients and family members on disease management, medications, and home care techniquesCollaborate with physicians, therapists, and care team members to ensure coordinated patient careMaintain accurate and timely clinical documentation in compliance with regulatory standards Requirements: Active RN license in the state of VirginiaOasis and SOC Experience a preferred and favoredPrevious home health, med-surg, or community nursing experience preferredStrong clinical assessment and critical thinking skillsExcellent communication and patient education abilitiesReliable transportation and valid driver's license for travel throughout the NOVA regionAbility to work independently and manage a flexible patient caseloadCompassionate, patient-centered approach to careMust commit to working a minimum of 3 days a week Mon/Wed/Friday About Us: Talin Home Healthcare LLC is a trusted home health provider based in Sterling, VA, committed to delivering exceptional, patient-centered care to individuals throughout the Northern Virginia community.
