Full Time Home Health Physical Therapist

What You'll Be Doing

• Conduct comprehensive physical therapy evaluations and create personalized treatment plans to help patients achieve their goals
• Teach patients and their families how to properly perform exercises and activities to promote recovery and improve mobility
• Document patient progress, including changes in condition, treatment plan, and goals, and communicate updates to other healthcare professionals
• Cook up care with other medical professionals, including physicians, nurses, social workers, and speech therapists, to ensure the most effective treatment is being delivered
• Adhere to established policies and procedures, including infection control and safety protocols
• Educate patients and their families on injury prevention and self-care techniques to reduce the chances of relapse or recurring injuries
• Maintain accurate and thorough records of patient interactions and treatment plans in accordance with state and national regulations

Requirements

• Graduate from an accredited physical therapy program with a valid Physical Therapy license in the state of Florida
• Minimum of one year of experience in physical therapy, preferably in a home health care setting
• Demonstrated ability to create effective physical therapy treatment plans that promote patient recovery
• Effective communication skills, both verbally and in writing, to effectively communicate with patients, families, and other healthcare professionals
• Ability to work independently and collaboratively as part of a larger healthcare team
• Ability to read and interpret physician orders and implement treatment according to orders
• A passion for helping patients achieve their best possible outcomes, through compassionate care and evidence-based treatment
