Full- Time Field LVN for Highland Park Senior Care Home Health - Longview, TX

About the role

We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team as a Field LVN in Longview, TX 75601. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their health and comfort while promoting independence and well-being.

Responsibilities

• Provide skilled nursing care to patients in their homes according to physician orders and care plans.
• Monitor patient health status and report any changes or concerns to the supervising nurse or physician.
• Administer medications, injections, and treatments as prescribed.
• Assist patients with activities of daily living (ADLs) as needed.
• Document patient care accurately and timely in compliance with agency policies and regulatory requirements.
• Educate patients and their families on health management, medication administration, and disease prevention.
• Collaborate with healthcare team members to coordinate patient care and ensure continuity.
• Maintain patient confidentiality and adhere to HIPAA regulations.

Qualifications

• Current and valid LVN license in the state of Texas.
• Minimum of 1 year nursing experience preferred, preferably in home health or related field.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively in a field setting.
• Reliable transportation and valid driver’s license.
• CPR certification preferred.

Work Environment

This position involves traveling to patient homes within the Longview, TX area. The Field LVN will work independently in a variety of home settings and must be adaptable to different environments and patient needs.
