Responsibilities
- Engages with staff routinely.
- Builds rapport with staff.
- Communicates effectively with CLNM staff and any outside entities.
- Manages time effectively while coordinating care for several clients at a time.
- Empathizes, is compassionate, and friendly with both clients and staff.
- Facilitates communication and information with the Director and Clinical Supervisors of CLNM.
- Collaborates with Outpatient MHR Staff in a collaborative and integrated way.
- Deals with complaints from all parties and addresses them as needed or refers to the proper party (MHR/CLNM organizational chart).
- Works with the QI Director to develop and improve quality measures and improve the performance of the program.
- Runs reports and disseminates them to the steering committee.
- Supervises an assigned team of care coordinators.
- Moves well through Electronic health records.
- Prioritizes multiple tasks at one time.
- Documents coordinated activities through BHSD Star and EHR.
- Utilizes community resources (food vouchers, housing, etc.) to help members improve their day-to-day living and extend their quality of life.
- Identifies risk factors and looks into the member's past and present issues concerning both behavioral and physical issues.
- Maintains HIPPA and client confidentiality at all times.
Requirements
- Bachelor’s degree required.
- Care Coordination experience strongly encouraged.
- Bilingual/Spanish speaking strongly encouraged.
Qualifications
- Full-time, 40 hours per week, exempt position.
- Starting pay: $45,900.
