About the role
We are seeking a full-time Care Navigator to support patient and family wellness through advocacy, assessment, planning, communication, education, resource management, and service facilitation.
Responsibilities
- Assist families in accessing medical specialty care, community services, developmental assessments, and school planning.
- Build trusting relationships via phone, text, in-person visits at clinics, homes, schools, or community spaces.
- Serve as the primary contact, advocate, and resource for patients, families, and community partners.
- Collaborate with healthcare providers to identify patients with complex needs and initiate care plans.
- Create and maintain culturally responsive resources and ensure accurate documentation.
- Uphold client dignity, privacy, and respect in all interactions.
Qualifications
- Bachelor’s degree in social work, psychology, case management, advocacy, or related field, or equivalent healthcare experience.
- Experience working in medical settings, reviewing medical records, and applying health principles.
- Strong verbal and written communication skills.
- Problem-solving ability and innovative thinking.
- Bilingual in Spanish/English preferred (additional signing bonus available).
