About the role
The Family Treatment and Rehabilitation Case Planner at Forestdale works with families facing challenges that endanger their children's safety and well-being. This role involves visiting families, providing counseling, advocating for services, and coordinating with various service providers.
Responsibilities
- Visit families in their homes, communities, or at the agency as needed.
- Provide individual, family, and group counseling as needed.
- Advocate for clients in securing services like public assistance, health insurance, and addressing immigration issues.
- Coordinate and maintain contact with workers at the Administration for Children’s Services.
- Maintain contact with schools, mental health facilities, therapists, and other service providers.
- Participate in Clinical Diagnostic Team Meetings, staff meetings, in-service trainings, and outside trainings for professional development.
- Document all case activity using the systems of record - N.Y.S. Connections and the N.Y.C. PROMIS.
- Absorb and respond to emergencies as needed.
- Ensure compliance with all agency policies, procedures, and governmental regulations.
Requirements
- Bachelor’s or Master’s degree in Social Work or a related field from an accredited institution.
- At least 1-2 years of experience in case management, counseling, and advocacy services.
- Some knowledge of the child welfare system, preferable.
- Strong organizational, written, and verbal communication skills.
- Ability to accommodate a flexible work schedule.
- Bi-lingual ability preferred.
Qualifications
- Bachelor’s degree: $29.12/hr - $31.04/hr; $30.22/hr - $32.15/hr (Bilingual); $30.22/hr - $33.24/hr (Masters); $31.59/hr - $34.34/hr (Masters (Bilingual)); $34.43/hr - $37.09/hr (Masters (Bilingual & License))
