FT Wound Care LP/VN with Hyperbarics

About the Role

The Wound Care Nurse-LPN is responsible for the assessment, planning, implementation, and evaluation of wound care. This role provides direct patient care and education, and is actively involved in departmental, transdisciplinary team, and LifeCare activities to ensure individualized, patient-centered healthcare for all admitted patient populations.

This position involves contact with adult (18-65 years) and geriatric (65 and older) patients in a clinical setting.

Responsibilities

• Under the supervision of the Wound Care Coordinator RN or MD, assess the needs of patients with basic and complex wounds, identify potential or actual needs, and communicate changes in patient status to the assigned nurse and physician.
• Document assessments and initiate/update the Skin Care Algorithm for all wound care patients.
• Develop and document a comprehensive wound management plan, assist staff in wound care, and update the plan as needed.
• Complete daily rounds and update the transdisciplinary team and family on wound progress.
• Implement the plan of care by providing wound care interventions directly or delegating care appropriately, demonstrating competent and proficient nursing practice.
• Respond to emergencies calmly and effectively, and assist others as needed.
• Evaluate wound care provided, document patient responses, and coordinate with nursing and other team members.
• Obtain and document photographs of wounds upon admission and weekly thereafter.
• Identify and address individual learning needs of patients and families regarding wound care education.
• Communicate changing patient needs to the assigned nurse and physician, ensure patient safety, and initiate/maintain appropriate safety measures.
• Oversee and direct delegated functions of CNAs and Patient Care Technicians.
• Adhere to LifeCare policies, procedures, safety plans, and regulatory standards, with regular attendance and timeliness required.
