FT - Noc Shift CNA-12 Hour Shift

Provide routine daily nursing care and services to assigned residents in accordance with their assessment and care plan, under the direction of supervisors.

Responsibilities

• Identify residents using wristbands or photo cards before administering treatments or serving meals.
• Use only authorized facility abbreviations when recording information.
• Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
• Record entries on flow sheets, notes, and charts in an informative and descriptive manner.
• Report all accidents and incidents observed during the shift.
• Maintain confidentiality of resident’s protected health information; report suspected or known violations.
• Report unauthorized attempts to access the facility’s information system.
• Perform only trained nursing care procedures.
• Prepare resident rooms for new admissions (e.g., bed made, name tags, admission kit available).
• Greet residents, escort them to their rooms, and introduce them to roommates and staff.
• Inventory and mark resident personal possessions; assist with packing for transfers or discharges.
• Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
• Maintain a warm, personal, and calm environment for residents.
• Meet regularly with nursing personnel to identify and address problem areas or improve services.
• Report all resident complaints and grievances.
• Participate in and receive nursing reports as instructed.
• Follow established policies for exposure to blood/body fluids.
• Perform routine tasks such as making beds, measuring vital signs (TPRs), and weighing residents.
• Check on residents unable to call for help frequently; answer resident calls promptly.
• Assist residents with meals (e.g., identifying food arrangements, recording intake).
• Keep water pitchers clean, filled, and within reach; perform after-meal care.
• Complete all assigned tasks according to established policies and procedures.
• Cooperate with inter-departmental personnel to maintain nursing services.
• Notify the facility of lateness or absence.
• Report occupational exposures to blood, body fluids, or hazardous materials.
• Assist residents with daily functions (e.g., dental care, bathing, dressing, grooming).
• Change wet or soiled gowns, clothing, and linens promptly.
• Assist residents in preparing for medical tests or examinations.
• Lift, turn, move, position, and transport residents using proper techniques and equipment.
• Perform restorative and rehabilitative procedures as instructed.
• Check residents routinely to ensure personal care needs are met.
• Observe and report pressure areas, skin breakdowns, or injuries of unknown source.
• Monitor disoriented and comatose residents; record and report data as instructed.
• Perform special treatments as instructed.
• Report any resident leaving or missing from the facility immediately.
• Use only equipment you are trained to operate; report defective equipment.
• Honor resident’s refusal of treatment and report such requests to supervisors.
• Report allegations of resident abuse or misappropriation of property.

Requirements

• Minimum 10th grade education.
• Ability to read, analyze, and interpret professional journals, technical procedures, or regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to present information and respond to questions from managers and employees.
• Apply mathematical concepts (fractions, percentages, ratios) to practical situations.
• Solve practical problems with limited standardization; interpret instructions in various forms.
• Licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification preferred.

Physical Demands

• Occasionally lift and/or move up to 25 pounds.
• Prolonged use of desktop or laptop computer.
• Regularly required to sit, stand, walk, talk, read, or hear.
• Frequent use of office equipment (copier, scanner, fax, telephone, calculator).
• May assist in evacuating residents during emergencies.

• Noise level is usually low to moderate.
