Foster Care Case Manager

About the role

Pennsylvania Mentor, a part of the Sevita family, provides Residential and Treatment Foster Care services to youth facing various out-of-home placements, transitions, or involvement in the juvenile justice system. Our personalized programming aims to help youth gain stability, comfort, and confidence to overcome challenges and thrive.

Responsibilities

• Provide support and resources to children and foster families.
• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.
• Aid in the development of quality services and engaging activities that meet the individual served needs.
• Build and maintain relationships with families and external case managers.

Qualifications

• Bachelor’s degree in human services or related field.
• One year of work-related experience working with youth placed at-risk.
• Bi-lingual English-Spanish welcome.
• Current driver's license, car registration, and auto insurance.
• A reliable, responsible attitude and a compassionate approach.
