Foster Care Case Manager

About the role

As a Foster Care Case Manager at Georgia HOPE, you'll play a critical role in supporting children and families within the foster care system. You'll ensure services are well-coordinated, plans are individualized, and every child's needs are met with consistency and care.

Responsibilities

  • Develop and implement individualized treatment and service plans that support each child’s safety, stability, and well-being
  • Coordinate medical, educational, behavioral health, and social services to ensure seamless, uninterrupted care
  • Serve as a central point of communication between foster parents, biological families, service providers, and internal teams
  • Facilitate and support healthy interactions between children, foster families, and birth families to promote positive outcomes
  • Maintain accurate, timely documentation and case records in accordance with organizational and regulatory standards
  • Partner with foster parents to provide guidance, support, and resources that help create safe, nurturing home environments
  • Collaborate with internal teams and external agencies to ensure coordinated service delivery and compliance with all requirements

Requirements

  • Bachelor’s degree in social work or a related human service field is required
  • A minimum of two (2) years of relevant experience is required

Qualifications

  • Strong organization, adaptability, and the ability to manage multiple priorities in a dynamic environment
  • Ability to communicate clearly, problem-solve proactively, and maintain attention to detail
  • Experience supporting foster parent recruitment, assessment, and ongoing engagement

Skills

  • Excellent interpersonal and communication skills
  • Strong organizational and time management skills
  • Ability to work independently and as part of a team
  • Knowledge of foster care systems and best practices

Pay

Compensation is commensurate with experience.

Schedule

Hours are flexible to accommodate the needs of the foster care system.
