About the role
As a Foster Care Case Manager at Health Connect America, you'll play a critical role in supporting children and families within the foster care system—ensuring they receive the coordinated, compassionate care they need to thrive.
Responsibilities
- Develop and implement individualized treatment and service plans that support each child’s safety, stability, and well-being
- Coordinate medical, educational, behavioral health, and social services to ensure seamless, uninterrupted care
- Serve as a central point of communication between foster parents, biological families, service providers, and internal teams
- Facilitate and support healthy interactions between children, foster families, and birth families to promote positive outcomes
- Maintain accurate, timely documentation and case records in accordance with organizational and regulatory standards
- Partner with foster parents to provide guidance, support, and resources that help create safe, nurturing home environments
- Collaborate with internal teams and external agencies to ensure coordinated service delivery and compliance with all requirements
Qualifications
- Bachelor's Degree required — in Human Services with a minimum of two years of experience working with youth or families, or in a non-Human Services field with a minimum of four years of experience working with youth or families
- Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis
