Responsibilities

  • Provide direct outpatient primary care, including diagnosis, treatment, and documentation per protocols.
  • Conduct patient assessments, prescribe medications, and order labs or imaging as needed.
  • Manage chronic diseases, support patient self-management goals, and monitor progress.
  • Collaborate with care teams, participate in case reviews, and serve as a mentor/preceptor.
  • Engage in continuing education, quality improvement, and patient-centered health home initiatives.

Requirements

  • BLS Certificate from American Red Cross or American Heart Association.
  • Full licensure to practice medicine by the NC Board of Nursing or NC Board of Medical Examiners, with valid license.
  • National certification as a Nurse Practitioner or Physician Assistant.

Preferred Qualifications

  • Minimum of one (1) year of experience.
  • Experience with underserved or rural populations.
  • Familiarity with team-based care or PCMH/PCHH models.
