Facility Operations
- Open and close the fitness facility according to approved hours of operation.
- Answer phone calls and provide front-line customer service.
- Inspect all fitness equipment routinely, perform preventative maintenance, and make minor repairs according to manufacturer's guidelines.
- Enforce fitness facility policies, procedures, rules, and regulations.
Client Services and Fitness Programming
- Orient clients to the facility and equipment; assist with safe exercise technique, body mechanics, and basic physiology.
- Monitor and evaluate participant progress; tailor interventions to client needs and limitations.
- Screen individuals with pre-existing medical conditions prior to exercise.
- Develop individualized fitness plans for referred patients in alignment with case management goals (BMI, A1C, weight management).
- Provide feedback, adjust plans as needed, and support patients in overcoming barriers (transportation, scheduling, motivation).
- Help create culturally appropriate activity programs and special events tailored to age and physical capability.
Referral & Case Management Coordination
- Receive and acknowledge patient referrals from case managers and fitness monitor leads.
- Contact referred patients within required timelines, encourage participation, and schedule orientations.
- Document all referral activities (contact attempts, participation status, barriers identified).
- Maintain and update the gym referral spreadsheet; ensure accurate and timely records.
- Participate in coordinated follow-up tracking, ensuring compliance with SOPs.
- Provide timely updates to fitness leads and case managers regarding referral outcomes and patient engagement.
Reporting & Program Support
- Assist lead fitness technician with data collection for monthly reports.
- Contribute data for KPI and QI reporting, including patient participation rates, follow-up outcomes, and progress toward health goals.
- Assist in development and advertisement of fitness facility services (brochures, flyers, calendars, etc.).
- Utilize current health, fitness, and medical knowledge to guide services.
- Assist with monitoring compliance to ensure fitness services align with program SOPs and standards.
- Perform other duties as assigned to maintain or enhance department operations.
