Fiscal Intermediary/ Self-Directed Support Staff

Qualifications

• High School Diploma or GED with one-year experience working with developmentally disabled.
• Valid acceptable New York State Driver’s License.
• Authorized to work in the United States.

Essential Functions

• Implement goals outlined in the Community Habilitation Staff Action plan.
• Maintain records of the individuals being served in the community Habilitation program and provide them to the Home and Community Services Supervisor weekly.
• Observe person supported for any unusual physical or behavioral changes and report to supervisor.
• Attend and satisfactorily complete all required training as assigned.
• Demonstrate flexibility, accuracy, and confidentiality in work assignments.
• Maintain confidentiality of all work related information.

Work Environment

Personalized home environment based on the person who resides in the home.

Physical Demands

• Regularly required to talk or hear.
• Frequently required to stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms.
• May require lifting, pushing, or pulling in excess of 30 pounds.

Wage Range

$15.20-$23.00/ Hour, based on the family's budget.
